Step 1: Install the Plugin

At first, go to the WordPress Dashboard > Plugins > Add Plugins. Then, search for “Bit Form” > Install the plugin.

As this plugin is a drag-and-drop form builder, you can drag any fields from the left sidebar to the form. So, I have dragged a section here. In this section, I added the following fields: Image, Title, Phone Number, and Address.

If you want to change the field name, you can edit the label and change the text. And to add an image, drag the image field > Background Image > Browse. Then you can add your preferred image to the form.

Here, I will create the form with a total of five sections. So now l have dragged another section to add the bakery items.

In this section, I added three image select fields: Cakes, Bread, and Cookies. And between each field, I added a “divider”.

And, to add the image of bakery items, click on the “Add/Edit” option. Then customize the label and add item images. Now, I have set a value for bakery items so that the price for each item can be shown accordingly.

Step 5: Add Total Price and Customer Information

Here, I have added two sections for the total price calculation and customer information. In the “Customer Information” section, I added the following fields: First Name, Email, and Phone Number.

Step 6: Add Delivery Information

Here, I added a section for “Delivery Information”. In this section, I added a dropdown option where the audience can choose between store pickup or home delivery. And other fields are – Delivery Options, Delivery Address, Date, Preferred time for the delivery, and Special Instructions.

Step 7: Choose a “Theme”

Step 8: Design the Form

This conditional logic will show the “Delivery Address” field when a customer chooses the “Home Delivery” option. And if they choose “Store Pickup”, then this field will be hidden. That’s why you have to hide the “Delivery Address” field in the form builder.

This conditional logic is for total price calculation. So, when a customer selects one or two items together, they will see the total price based on the total number of items.

Step 11: Update and Publish

Now, click on Update > Publish. After clicking on Publish, you will get a short code. You need to copy and paste the shortcode in the WordPress post section. Then, you can test your form.
